When people with multiple needs face poor health outcomes, neglect and systemic barriers, the answer is not another placement into another inpatient or congregate setting. Nor should discharge be delayed while waiting for long-term community support.
The only thing people need is person-centred care in a safe and calming environment. People need to feel compassion and genuine affection. Care teams who care about them, not for them. The system made us believe there is rarely time to secure the ideal long-term solution. So, the real question becomes: do we accept this narrative or create a better path forward?
At Leaf Complex Care, finding new pathways is our mantra. We provide specialist, long-term community care and support for people with multiple needs in complex situations, primarily for autistic people and people with a learning disability. Our care teams develop person-centred solutions that allow people to avoid unnecessary hospital admissions. We also have a Rapid Response Service for crisis situations, delivering tailored support within 4-6 hours of the initial call, ensuring timely intervention, prioritising stability and dignity.
Our support is designed on the grounds of PROACT-SCIPr-UK®, the Capable Environments model and the PERMA model of well–being for people who require:
- A bridge to long-term solutions, whether transitioning home or supported living.
- Structured, person-centred caregrounded in Positive Behaviour Support (PBS) to manage underlying triggers
- Multidisciplinary expertise, including PBS specialists, occupational therapists, and highly trained support workers
- Immediate crisis intervention to de-escalate distress and prevent harm.
For commissioners, social workers and other care professionals, we offer a flexible and trusted partnership through co-production, integrated care and a *new transparent and cost-effective model of care, ‘The Thrive Care Pathway’.
